Here at TLC Chiropractic, we value our patients. We acknowledge that going to a new place for the first time can be nerve-racking. We want to do everything we can to eliminate any unnecessary stressors, which is why we include the option of printing and completing your forms at home. We think of our patients as part of our extended family, and we are excited to meet you!

New Patient Form

What is The Process Like?

Step One

All new patients are required to fill out the new patient health application form which we will provide for you when you come in to get checked.

Step Two

Next a one-on-one consultation will be conducted to discuss your health conditions and your overall health goals. This consultation will provide us with a better understanding of your current health status, locate the cause of your health conditions, and properly address your needs.

Step Three

Then we begin our evaluation procedures. These assessments are focused on locating and correcting Vertebral Subluxations. The procedures we use in our office focus on your spine and nervous system.

Our comprehensive chiropractic evaluation and neurological diagnostic testing are performed to further determine the exact cause of your health conditions. Using our state-of-the-art diagnostic equipment, we identify and locate the dysfunction within the body. Read more about this in the Chiropractic Evaluation section below.

Chiropractic Evaluation

Our Evaluation includes:

  • Posture Evaluation
  • Digital X-Rays (if necessary)
  • Thorough History
  • Range of Motion Assessment

These results provide us with a better understanding of your current state of health. These findings, along with their significance, will be discussed and explained during your Report of Findings appointment.

Step Four

After the initial evaluations, Chiropractic x-rays may be required (this is based on individual assessment results). X-rays are an important component of the examination process. Postural x-rays allow us to evaluate the structure of the spine and help to locate vertebral subluxations. They also give information relative to the position of the misaligned bone and the extent of spinal degeneration.

At TLC Chiropractic, we firmly believe, “to see is to know, and not to see is to guess”!

All this gathered information allows our doctors to make an informed and accurate determination for the type and duration of chiropractic care. Your results are explained in detail during your Report of Findings appointment.

Step Five

Your Report of Findings appointment is the most important and is scheduled within a few days of your initial visit. This is a mandatory appointment.

Our doctors take time to discuss with you the cause of your health concerns. They will help you determine if chiropractic care is right for you and if it is, the parameters they recommend for your care.

At TLC Chiropractic, we provide accurate information concerning your health through patient education and providing answers to your questions. We want to partner with you in improving your health!
